LUMERIN MIND & BODY SESSION LIABILITY WAIVER & RELEASE

IMPORTANT – PLEASE READ CAREFULLY BEFORE PARTICIPATING IN ANY SESSION

By signing this document, you acknowledge and agree to the following terms and conditions, releasing Erin (the “Facilitator”) and Lumerin (the “Business”) from any and all liability in connection with participation in any services or offerings.

1. VOLUNTARY PARTICIPATION

I understand that my participation in any Lumerin session, including but not limited to breathwork (including 9D Breathwork), meditation, sound therapy, energy healing, or wellness coaching, is completely voluntary.

2. NOT MEDICAL OR MENTAL HEALTH CARE

I understand that these services are not a substitute for professional medical or psychological treatment, diagnosis, or advice. I confirm that I will consult my physician or mental health provider if I have any concerns prior to participating.

I affirm that:

• I am not experiencing any condition that would impair my ability to safely participate in breathwork or related activities.

• I will disclose any relevant physical, mental, or emotional health conditions prior to the session.

3. ASSUMPTION OF RISK

I understand that breathwork and related modalities can involve intense physical, emotional, and mental experiences. I acknowledge the potential risks, including but not limited to:

• Dizziness, fainting, hyperventilation, emotional discomfort, muscle tension, or nausea.

• Emotional release or unexpected psychological responses.

I assume full responsibility for any risks or consequences that may arise from participating in any session with Erin, whether conducted in-person or online.

4. RELEASE OF LIABILITY

I hereby waive, release, and discharge Erin [Last Name], Lumerin, and any of their employees, agents, contractors, or representatives from any and all liability, claims, or demands — known or unknown — arising out of or connected with my participation.

This release includes, but is not limited to:

• Personal injury (physical or emotional)

• Medical incidents

• Psychological trauma

• Property damage

• Any consequences resulting from my participation or the use of any recommendations or practices suggested during or after the session

5. ONLINE/REMOTE SESSIONS

I understand that for virtual sessions:

• It is my responsibility to ensure a safe, quiet, and undisturbed environment.

• I will not operate machinery, drive, or engage in risky activities during or immediately after a session.

• Lumerin and Erin are not liable for accidents or outcomes resulting from my environment or actions during an online session.

6. CONFIDENTIALITY & RECORDING

I agree not to record any part of the session without explicit written consent. I understand Erin may take notes for internal use but will not share personal information without my written permission, unless required by law.

7. AGREEMENT & UNDERSTANDING

By signing this document, I confirm:

• I have read and understood this entire waiver.

• I am of sound mind and legal age (18+), or I have legal guardian consent.

• I voluntarily agree to these terms and release Erin and Lumerin from any and all liability.
